Causes of illness according to Ayurveda

Ayurveda is a system of lifestyle. In Ayurveda causes of diseases is mentioned as less, more and wrong unison of time, senses and functions. These all lead to tri doshas imbalance in the body and disease occur.

Hina (less), ati (more) or mithya (wrong) unison of Kala (time), Artha (senses) and Karma (functions) is the reason for diseases and the right unison of these three factors is the reason for health. Ayurvedic treatment is based on individual’s doshas (Vata, pitta & Kapha ) constituents or prakriti, their state of imbalance, lifestyle, diet, mind state and environment.

The goal of ayurvedic treatment is to restore the balance state of the three doshas, through personalized treatment plan and therapies, diet and lifestyle modifications addressing the root cause of the disease. Piles / Haemorrhoids treatment

In Ayurveda piles is called "Arsha". It means like enemy. Piles are like enemy which hurts us so much and disturbed our daily routine. Piles are swollen and inflamed veins in the anus. Lump outside or inside the anus is common symptoms of piles. They are common and can affect people of any age.

There are two types of piles- internal and external.

Internal Piles – swelling inside the internal anal sphincter, located inside the rectum, usually painless, may cause bleeding during bowel movements, it may be of 4 grades based on size and position.
  • First grade - lump inside the anus a bit higher than anal opening and can be found only on examination by doctor
  • Second grade – size get enlarged and comes down to the anal opening, you may feel discomfort inside the anus
  • Third grade - lump comes out of anus during defecation; initially lump goes inside itself after defecation and later it has to push inside with finger
  • Fourth grade - enlarged lumps remain outside the anus, often bleed

    • External piles – located under the skin around the anus, can be painful, may cause swelling, itching and discomfort

    In Ayurveda piles is classified according to prominent doshas. It may be of vataj, pittaj, kaphaj or combined.

Common Symptoms of Piles:

Itching, burning pain and discomfort around anus, fresh bleeding during defecation, lump around anus, hard stool or constipation.

Causes of Piles:

Piles develops mostly due to increased pressure in the lower rectum, common causes are – Chronic constipation, straining during bowel movements, sitting for long periods, low fiber diet, pregnancy, obesity, heavy lifting, chronic diarrhoea, hereditary, etc.

Diagnosis:

History of anal pain and discomfort, itching anus, lump in anus, bleeding per rectum, etc.

Physical examination of anus:

  • Examination by Proctoscope to check the internal piles
  • Digital examination to feel enlarged piles mass

Treatment:

Ayurvedic treatment of piles aims to restore the balance of doshas (vata, pitta and kapha ) and improve agni (digestive fire). Ayurvedic treatment follows holistic approaches including dietary changes, lifestyle modifications, herbal remedies and minimal invasive procedures to address the root cause and prevent recurrence.

Medical Treatment:

Piles in the initial stage are treated with diet restriction and medicine. Ayurvedic medicines are effective in piles treatment. There are popular herbal remedies for piles treatment such as trifala, abhayarista, arshakuthar ras, arsogni bati, jatyadi tail etc. Ayurvedic practitioner prescribes single and compound formulation to treat the disease.

Surgical Treatment (Minimal Invasive Procedure) :

If the piles lump is bigger in size, third and fourth degree, bleeding piles we treat it with kshyarkarma and Kshyarsutra (medicated thread). Kshyarkarma - Kshyar or alkaline herbal paste derived from different herbs is applied to the internal haemorrhoid to shrink and obliterate it.

Kshyarsutra - The procedure is Kshyarsutra ligation (KL) of the enlarged piles mass. Qualified Ayurveda surgeon or doctor tie up the base of piles mass with Kshyarsutra. Kshyarsutra has properties of curettage and healing. First it blocks blood flow, then it curettage the base of piles mass and helps to heal. Kshyarsutra would minimize the chances of bleeding and infection of the wound. This is very simple technique which can be done in outpatient department with aseptic precautions. Kshyarsutra treatment is done with local anaesthesia. After procedure the wound is taken care for 7 days. Mostly it falls off in 7 days with thread and shrunk piles mass. There is less chances of bleeding. Patients complain of pain during the first 3-4 days then it gradually vanished in 7 days. Kshyarsutra treatment of piles in Ayurveda is most popular and effective technique. This is cost effective, convenient and non-relapsing technique.

Life Styles Modifications:

  • Increase fiber intake – fruits, vegetables, whole grains
  • Avoid spices- chilli, ginger
  • Avoid "maida" and black pulses
  • Drink plenty of water (2-3 liters/day unless restricted)
  • Avoid straining during bowel movement
  • Avoid prolong sitting on the toilet or any sit
  • Regular yoga and exercise

Home Remedies:

  • Sitz bath with potash and lukewarm water, 2-3 times a day for 10 minutes
  • Ice pack or cold compress
  • Rest

  • Prolapsed lump outside the anus
